The National Disability Insurance Scheme (NDIS)
The NDIS is funding for people living with a disability, their families and carers to help them achieve personal goals and improve life outcomes. For those eligible, the NDIS means gaining greater choice and control over their disability services.
Through the NDIS assessment and planning process, you can determine what support you need to live your day to day life and to achieve your personal goals.
How do I apply for SydWest services?
If you are a person with disability and you meet the NDIS access requirements you can become a participant in the scheme. We can provide personalised advice and begin your journey towards a brighter future.
All referrals will be responded to within 48 hrs.
Creating your NDIS Plan
Our Planners will:
- Help you to understand and navigate the NDIS.
- Assist you to create your personalised NDIS plan based on your personal needs and goals.
- Work with you to achieve your goals, implement your plan and link you with the right people, products and organisations within your local community.
